- 1. JAMB Result Slip
- 2. OL Result
- 3. Certificate of Local Government of Origin
- 4 Two (2) Recent Passport Photographs
- 5. The Application Form(photocopy) and Bursary receipt
- 6. Birth Certificate Jamb Result
